DESCRIPTIONUnder the supervision, direction, and guidance of the RN, the Emergency Department Inpatient Transition Clinical Care Partner (CCP) provides patient-specific observation, care, and intervention as directed by unit protocol to at-risk patients of the Emergency Departments and Inpatient Psychiatric Departments. The CCP collaborates with the patient care team in the formulation and application of the patient plan of care. Demonstrates knowledge of therapeutic interventions and crisis interventions applied to patients that are cognitively and/or psychiatrically impaired. Intervenes as necessary to reduce and prevent self-harm. Assists with other related clinical environments and clinical support functions as required.
